At this time, we are prioritizing individuals joining ChapelConnect(CC), our annual membership program that supports improved services such as a full-time clinic nurse, updated technology, email reminders, and online booking.​

 

ChapelConnect fee is $249.99 + HST per year (annual renewal)

To join the ChapelConnect waiting list, please visit the clinic to complete a form in person.

A separate list will be maintained for non-ChapelConnect applicants; however, these individuals may experience a significantly longer wait.
